We want to thank all of you who were able to join us for our Regional Learning Collaborative Event last Friday, March 24, 2017. We had a great turn out.

We appreciate all of the evaluation forms that were completed. At a quick glance the feedback looks very positive with majority being satisfied to highly satisfied. We will review the details on the evaluation forms and implement improvement ideas for our September event.

RHP 12 DSRIP in Action-Swisher Memorial Healthcare System, Tulia, TX-Primary Care Expansion and Enhance Urgent Medical Advice-Recap .

Regional Healthcare Partnership 12 along with Swisher Memorial Healthcare System in Tulia, TX hosted our February DSRIP in Action event last Friday, Feb. 24th. Hosts for the event were Ryan Barnard, CEO, Swisher Memorial Healthcare, Elizabeth Adams, BSN, RN, Director of Quality & Foundation, Swisher Memorial Healthcare Strategic Initiatives, Brad Roberts, Executive Director of IT and Tulia Rural Health Clinic, and Kelly Maxwell, BSN, RN, CPN, Chief Nursing Officer.   There were 13 attendees representing 8 different providers from our region.

Elizabeth Adams and her team presented two of their projects, Primary Care Expansion and Enhance Urgent Medical Advice. The Primary Care Expansion has increased access to healthcare in the primary care setting, decreased the utilization of the Emergency Department for non-urgent care and provided patients with the right care in the right setting while controlling costs. Swisher has been successful in implementing a Medical Advice Line by providing a FREE 24/7 Medical Advice line, decreasing the utilization of the Emergency Department for non-urgent care and providing our patients with the right care in the right setting while controlling costs.

RHP12 DSRIP in Action - TTUHSC-Lubbock, TX- Nurse on Demand & Patient Navigation -RECAP

Regional Healthcare Partnership 12 along with Texas Tech University Health Sciences Center in Lubbock, hosted a DSRIP in Action event on Thursday January 19, 2017. Hosts for the event were Paul Fowler, Senior Director, Office of Strategic Initiatives, and Wendy Forrest, Program Director, Office of Strategic Initiatives. There were 30 attendees representing 14 different regional provider locations at the event.

Wendy Forrest and her team presented two of their projects, Nurse on Demand and Patient Navigation.Nurse on Demand is an after-hours & weekend nurse Medical advice line for pediatric clinic patients and caregivers. The call center has had a total of 8,631 calls since the inception of program and they have had a total of 400 patients who attended scheduled PCP appointments and caller Ed Diversions percentages were 75% and above in 2016.

 TTUHSC’s patient navigation project helps to decrease unnecessary emergency room visits and hospitalizations by educating, empowering and serving as advocates for patients with chronic illnesses.From inception 325 patients have enrolled in this program. As of 12/31/16: 161 patients that are active in the program. During DY5 Emergency room visits for enrolled members decreased 37.74% and Inpatient Admissions by 34.23%.

RHP 12 Regional Learning Collaborative-March 24,2017

Please register for our upcoming RHP 12 Regional Learning Collaborative Event scheduled for Friday, March 24, 2017.  The event will take place in the McInturff Conference Center in Lubbock, Texas. Please see the attached Tentative Agenda- Registration and breakfast will begin at 8:30am. Please note this is a tentative agenda but the start time will not change.
