Project Overview:

Provider Overview:  Childress Regional Medical Center is a 39-bed hospital located in the city of Childress, Texas.  The hospital is the primary healthcare provider for 30,000 residents in a 5 county area of the southeastern Texas panhandle; all 5 counties are HPSA designated.

Project Description:  The palliative care team brings an interdisciplinary approach to care for patients.  Physicians bring expert skills in management of pain and other symptoms for patients as the end of life nears. They help families navigate decision making with regard to appropriate goals of care based on the individual prognosis for each patient.  This process educates patients about their individual prognosis and potentially assists in transitions to Hospice programs where appropriate.

Our palliative care program provides services to patients who are nearing end of life as well as to patients who aren’t yet home bound (home health criteria) and aren’t yet terminal (hospice criteria).  The target population is all patients who fall into either of those two groups: patients receiving cancer treatment and ER patients who might benefit from the program as well as all patients undergoing treatment for terminal illness, patients being treated for chronic conditions in poor control, and patients whose quality of life have deteriorated due to illness.

HHSC Waiver Renewal Page: Opportunity for Public Comment

HHSC has updated their Waiver Renewal webpage to include the 1115 Waiver Extension Application Draft with supporting documents.HHSC will be taking comments on this draft document during July at the statewide public meetings. HHSC will post a survey link on the Waiver Renewal website for stakeholders to provide feedback and comments on the waiver extension draft.

As a reminder the public meeting in RHP 12 will be held on Friday, July 24, 2015 at the Region 16-Educational Services Center (5800 Bell Street, Amarillo, TX  79109).  The meeting will start at 1:00 PM in the Lecture Hall.

3-Year Project Change Request for DY-5

HHSC has recently posted materials for the 3-Year Project Change Request.  The forms can be access below:

In addition HHSC host a webinar on July 2, 2015 to outline the process for this change request.  A recording of the webinar can be accessed by clicking here.  It is important to note, changes that reduce QPI or MLIU will be subject to a secondary review by Myers & Strauffer, and may result in project valuation being reduced.  The timeline for 3-Year Project Change Request is outlined below:

  • July 17, 2015 – Final date for anchors/ providers to submit questions regarding change requests to HHSC.
  • July 27, 2015 – Anchors submit completed Change Request Forms (CRFs) and revised project narratives to HHSC.
  • Early October 2015 – HHSC sends anchors/ providers feedback.
  • November 18, 2015 – Anchors submit responses to HHSC feedback.
  • Late January/ Early February 2016 – HHSC sends anchors/ providers final determinations.
