Below is the information on how to remotely participate in the upcoming Statewide Learning Collaborative summit in Austin that will be hosted by HHSC on August 27thand 28th .

HHSC has arranged for the Statewide Learning Collaborative Summit to be available for viewing on-line, in real time, in order to allow all interested individuals to watch the proceedings.  To access the Summit via webcast on August 27th and/or 28th, please click on the HHSC Webcast link. The webcast will only stream the sessions that are occurring in the Grand Ballroom.  Please see the agenda for Summit sessions.

RHP 12 Learning Collaborative-Sept 2015

The RHP 12 Regional Learning Collaborative Event is scheduled for Wednesday, September 23rd. The event will again take place in the McInturff Conference Center in Lubbock, Texas. Below you will find all of the information you need for registration, a tentative agenda, and poster presentations details.

-        Please Register Today. With the change in date for our event from August to September the Overton is unable to offer us discounted room rates. We are working to secure a discounted rate at another hotel and will send that information as soon as it is available.

-        Here is the Tentative Agenda.  Registration and breakfast will begin at 8 am. Please note this is a tentative agenda but one thing that will not change is that Registration and Breakfast will begin and be offered between 8:00-8:45 am.

-        Poster Presentations- We look forward to seeing many poster submissions. If you haven’t yet presented a poster of your project we would love for you to share at this event, please consider signing up.  We are again offering two poster options: “Template” style or “Freestyle”. Template style we design, create and print the poster for you. Freestyle you design, create and bring your poster to the event.

      • Please be prepared to stand by your poster during the poster breakout sessions
      • Please practice presenting your poster
      • Bring plenty of business cards

Template Style

    • Please complete the questionnaire/template with the information to be presented on your poster.
    • Send your questionnaire to This email address is being protected from spambots. You need JavaScript enabled to view it. no later than September 9, 2015.
    • UMC will take your information from the questionnaire/template you provide and will prepare your poster
    • Poster will be printed by UMC
    • Poster will be ready for display the day of the event

Freestyle

    • Here are some useful tips for preparing your posters
    • Be creative!
    • Present your project in your own style.
    • Please reference the questionnaire/template as your prepare your poster.
    • Bring your poster to Sandy James once you arrive at the even

July Monthly Status Call

Project Overview:

Provider Overview:  Childress Regional Medical Center is a 39-bed hospital located in the city of Childress, Texas.  The hospital is the primary healthcare provider for 30,000 residents in a 5 county area of the southeastern Texas panhandle; all 5 counties are HPSA designated.

Project Description:  The palliative care team brings an interdisciplinary approach to care for patients.  Physicians bring expert skills in management of pain and other symptoms for patients as the end of life nears. They help families navigate decision making with regard to appropriate goals of care based on the individual prognosis for each patient.  This process educates patients about their individual prognosis and potentially assists in transitions to Hospice programs where appropriate.

Our palliative care program provides services to patients who are nearing end of life as well as to patients who aren’t yet home bound (home health criteria) and aren’t yet terminal (hospice criteria).  The target population is all patients who fall into either of those two groups: patients receiving cancer treatment and ER patients who might benefit from the program as well as all patients undergoing treatment for terminal illness, patients being treated for chronic conditions in poor control, and patients whose quality of life have deteriorated due to illness.
