May 2016 Monthly Status Call

The May monthly status call is now posted.  To access, this call or any other monthly status calls click here.  The call provided a recap of our learning collaborative events from the first part of 2016.  In addition, the schedule for the cohort was discussed along with other upcoming LC events.  We highlighted the recently approved waiver extension and the next steps associated with the extension.  Stay tuned for more information related to the extension/renewal.  

RHP12 DSRIP in Action-Dimmitt, TX- Patient Navigation

We are excited for our next DSRIP in Action for 2016. Janet Sammann with Plains Memorial Hospital (Castro County) will share details on their Patient Care Navigation project. You will hear about educating patients they service with multiple chronic conditions, cognitive impairments and disabilities, limited English proficient patients, recent immigrants, the uninsured, those with low health literacy, and frequent visitors to the ED.
